We are looking for nurse practitioners to join our team who are committed to creating meaningful patient experiences. -As a hospice nurse practitioner at ACG Hospice, you'll provide patients with diagnosis, treatment, and care under the supervision of a physician. In this role, you'll focus on providing hospice care as an extension of a physician, Conduct complete physicals, provide treatment and counsel patients. Order and interpret test as indicated and recommend treatment of patients. Administer therapeutic procedures. Prescribe therapy or medication with physician approval.   A heart to serve patients and families and a passion for providing the best possible careEducation: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.Required: Reliable transportation.
